Chronic Disease: reduce prevalence and impact
One of our priorities for Vermont over the next decade through 2030 and beyond is to reduce the prevalence and impact of chronic disease.
Chronic disease has a significant negative impact on people and communities across Vermont. Preventing chronic disease requires more than just knowledge about healthy behaviors. People and communities need support to increase access to resources, and to improve the systems and infrastructure that give people the opportunity to make healthy choices.
